If your COVID at home/outside test is NEGATIVE, no more information is needed. Please follow the SHS/SON protocol for return to campus/class/work. You may submit your form without filling out the next section. If your test is POSITIVE, please answer the questions below for the Virginia Department of Health. Next quarantine and await contact from your department and COVID Team for further instructions.
