Proof of Enrollment

• Student’s can access the Enrollment Verification form in Self Service or the Centralized Student Assistance Office in Wiley Hall.  Return the completed form to the Registrar’s Office via email, fax or mail for your request to be processed.  

   

The Enrollment Verification form must be completed before a proof of enrollment letter or form is processed.
