When is the E&H “Immunization Record and Medical Information Form” due?

Please notify the Health Center if you have an extenuating circumstance that prevents you from submitting the form on time. Late forms will be accepted but if you are a non-compliant student, a hold may be placed on your class registration or residential housing or you may be unable to participate with your sports team.
